Contact the Bureau of Identification when you need the current release form.
Prepare two forms of identification
One must be a state-issued photo ID. The second can be another item displaying the person’s name.
Have the subject sign the waiver
A waiver/release should be completed and signed by the person whose record is requested.
Notarize forms completed outside the office
Forms not completed inside the Bureau of Identification must be notarized.
